    Well I did it today! I was tinkering with my driver lowering the loft a tad and backed the screw out a bit too far and the head came off while tightening back down I guess I cross threaded it on I can't get the shaft back on. Fortunately I have another shaft but not the one I prefer so I can still play but I need the other fixed now. So how do I go about getting the problem resolved do I send it back in or do I try to get someone to chase the treads? I also noticed while it was apart the rubber washer is missing. So I guess I need a new one of them as well so where can I get it also? Thanks.

    My brother had the same thing happen to him. In his case, the rubber washer was stuck inside the hosel on the club head (folded in half) and prevented the club head/shaft from tightening down correctly (acted just like it was cross-threaded). Try taking the head off again and see if the if the washer is stuck in the hosel, can't hurt to check.
    Hope it turns out to be something simple.

    You can try to chase the threads but worst case scenario you can take it in to your nearest golf pro shop and get the hosel removed and replaced with a new one i had mine done at golfsmith

    You can get any Titleist Retailer to send the shaft back and have the Sure-Fit adapter replaced. Titleist is the only place to get a genuine part, anyone else that does this will be using a knock-off adapter.
