this topic has come up many times, I had my name and golf club emblem embroidered on my bag at a local sign writers shop for £15, do a search for companies who do workwear embroidery, there's loads around in this area, and they can download various designs to meet your requirements

If it helps Look at local embroidery business they can do this for you I handed my front panel in and they are doing it this week and a good bit cheaper than companies that specialise in this I’ll post a picture of it when I get it back.

When I ordered my Premium Stand Bag from the pro shop I had them put the order into Titleist do my embroidery

Gentle reminder... Titleist will personalize when you order a new one from them. The do not customize after a bag has been delivered. Nice thing about having Titleist do it is a guarantee to get it right. If you send it to someone else to be done and they mess it up, Titleist does not sell replacement panels so you are pretty much stuck with it or you will have to get the embroidery company to replace the whole bag.
